Use this form to submit your comments or to request a Notice of Unpaid Toll resent to you. Specific comments or requests regarding your rental vehicle not pertaining to violations and/or toll-related matters should be directed to the location where you rented the vehicle.

If you need to dispute a recent violation received in the mail and/or charged to your charge card, please click here.

If you need to have a Notice of Unpaid Toll resent to you, provide below your name, rental agency location, contract number (if you have it), the date and amount of the charge on your statement (if you've been charged already), and a phone number if we need to contact you.

Most importantly, provide the address where you would like the Notice of Unpaid Toll sent. Please allow two weeks for delivery.
